Driver Authorisation Application. You will need this form to apply to Queensland Transport before you attend our training course. This download also includes the Commercial Vehicle Driver's Health Assessment (below) plus some information and instructions from Queensland Transport.

Commercial Vehicle Driver's Health Assessment. You will need to have a doctor (any General Practioner) complete this form. You will then need to lodge it with Queensland Transport at the same time as your Driver Authorisation Application.

Driver Location Study Guide. As part of your assessment at our training course, you will need to demonstrate a basic knowledge of Brisbane's major roads, suburbs and public buildings. This guide will help you study for the assessment.
